At Forest Hills Assisted Living, delicious and nutritious meals in our dining room provide a foundation for good health, while our special wellness program will keep you up to par. Senior excursions, exercise classes, activities and programs are available to increase your energy, fitness and knowledge. We also offer physical, occupational and other therapies on site, should you need them.

MAINTAIN YOUR INDEPENDENCE

At different times in our lives, we all need varying levels of care and support. Do you want someone to remind you to take your daily medication? Could you benefit from a little assistance with personal care? Do you need transportation to your doctor appointments? The level of support that is provided is all up to you. In addition, the proximity of round-the-clock nursing staff provides extra security for our residents. QUALIFICATIONS

• Maintain personal hygiene and appearance
• Ambulate by self, or with cane, walker, or wheelchair
• Transfer from bed or wheelchair without assistance
• Use a toilet safely
• Keep room clean and orderly
• Must not wander into others' rooms or off premises

*An assistant may be hired to sit or stay with a resident if necessary

Thorough evaluations are conducted with each candidate to determine if admission is appropriate.

• Studio apartment with kitchenette (small refrigerator and sink)
• One-bedroom apartment with full kitchen (range, refrigerator, and sink)
• Full carpeting, vertical blinds, and ceiling fans
• Individual heat/air controls
• 24-hour in-house staff
• 24-hour emergency call response
• Alarms and sprinklers
• All utilities except telephone
• Basic cable TV
• Scheduled transportation

COMFORTS AND CONVENIENCES

• Three meals daily
• Special diets when requested by a physician
• Tray service up to three days/month
• Weekly housekeeping and laundry
• Assistance with personal care, if needed
• Certified personnel to assist with medication
• Wellness Classes and Program
• Activities and Recreation, led by a professional Activities Director

• Barber/Beauty shop on premises (services must be purchased)
• Massage Therapy (services must be purchased)

ADDITIONAL CARE
